WEYMOUTH PERMISSION & RELEASE FORM for MINORS

Any event that involves the church taking children away from the church property without direct parental participation requires the signing of this permission & release form. An optional form is attached that can serve as a blanket permission for future events, as a convienience for regular participants.

APPLICATION FOR VOLUNTEERS SERVING with MINORS

We ask all of our regular volunteers for nursery, children's or teen ministries to fill out the attached application form, provide two friend references, and submit to a basic background check. Everything you'll need is attached to this download.
